Made to order when nothing on the standard list will do. You supply the measurements and the configuration, and the belt is built to that specification.
This route is worth taking for a client with a significant asymmetry, an unusual body shape, or a chair where the anchor points sit somewhere a standard belt cannot reach cleanly. It is also the answer when a previous belt has failed clinically and you know exactly what needs to change.
Custom builds carry a longer lead time than stock items, so bring it into the plan early, well before the client needs it.
