An arm that has lost active control needs a place to sit. A swivel arm trough supports the forearm and hand in a padded channel mounted to the wheelchair armrest, holding the shoulder in a neutral position and taking the weight of the arm off the joint.

That matters most after a stroke or with a flaccid upper limb, where the weight of an unsupported arm pulls at the shoulder and can lead to subluxation and pain. Supporting the forearm also keeps the hand from dropping into the wheel or the frame during a push.

The swivel action is for everyday use: the trough turns aside so the client can get close to a table or so a carer can bring them through a transfer without the trough being in the way, then returns to its set position afterwards.
